Initial Consultation

$125

 

Consultations consists of a lengthy client interview focusing on lifestyle, medical history, emotional tone, physical features, iridology and physical examination.  This initial visit is usually 60-90 minutes in duration, as such time is essential in learning more about you as an individual and in considering a strategy to help you meet your health goals..  Please visit our Forms page to complete the New Client Form prior to the initial consultation.  You will also need to review, print and sign the Notice of Privacy Practices, as well as the Minor Client Form, if applicable.  Where possible, it is requested that this form be submitted one week prior to the initial consultation so it may be reviewed prior to your visit.  This will enable us to make the most of your initial consultation.

 

 

Follow-Up Visits

$50

 

Follow up visits are approximately 30 minutes.

 

As a professional courtesy, please notify me at least 24 hours in advance if you need to cancel or reschedule an appointment.

How to Make the Most of Your Appointment

Payment

  1. Inform me of the reason for your appointment when scheduling.
  2. Complete the New Client Form prior to the Initial Consultation.  Also, be sure to review, print and sign the Notice of Privacy Practices and Minor Client Form, if applicable.  These may all be found under our Forms.
  3. Bring:
    1. Your supplements and medications (I am not looking to advise you regarding prescription medications.  However, it is imperative that I'm aware of them so as to avoid recommending anything that may have contraindications when mixed with particular drugs or other substances.
    2. A list of your health concerns and questions so you can remember all of your goals. Prioritize the list so we can cover the most important items first.
    3. Recent and/or relevant laboratory results that pertain to your health concerns.
  4. Prepare children for appointments by bringing along toys, puzzles, books, or homework. Most children will sit through an appointment if they are properly prepared.  Tell children how long you expect the appointment to last and your expectations of them.

 

Please note that all payments must be rendered prior to the consultation.

While we hope to have more options soon available for your convenience, we presently only accept payments in cash or via PayPal.

Due to the amount of time and preparation required for the initial consultation, payment must be rendered prior to forms being reviewed (no later than 48 hours prior to the initial consultation).

Follow-up visits may be paid for via PayPal prior to the initial consultation or in cash at the time of the consultation.
